Our middle school/junior high programs follow the TSSAA mandate requiring sports physicals be dated after April 15th of the same year that the school year starts.
All health forms must be completed before athlete can participate. Grades 5th-8th will use HealthyRoster to manage all health forms. COMPLETE REGISTRATION NOW, UPLOAD HEALTH FORMS LATER!!

HealthyRoster is an electronic medical record that allows our sports medicine professionals to manage all health forms for the current school year and effectively communicate with parents regarding injuries, treatments, doctors appts, etc. Parents, you will never have to hear "I don't know" from your athlete regarding an injury or a treatment plan - you will have access to view all information yourself and/or contact the trainer via the texting feature or video chat feature.
Once the TeamSnap registration has been completed, parents will receive an email from HealthyRoster with instructions. Parents will then be able to:
- Complete required health forms online, OR
- Download/upload completed forms
HealthyRoster allows you to enter health forms/info ONE TIME per school year regardless of the number of sports your daughter/son plays for SAA-SDS.

5TH-8TH Athletics
St. Agnes Academy - St. Dominic School provides an education-based athletics program that accompanies our student-athletes from early skill development to high level competition. Our core values of Commitment, Sacrifice, Work Ethic, Respect, and Pride, are woven throughout our program at every level. These core values are the basis for student-athletes to form strong relationships, build life-long skills and habits, and thrive within a team structure.
We view our athletic programs as an extension of the school day. Our coaches are educators in their respective sport. For some student-athletes, they may consider their sport one of their most important “classes” of the day. We believe student-athletes will become better persons through their participation in our programs. Student-athletes will experience “bumps in the road” along the way. Mistakes and adversity are part of the growth process. We will plan, prepare, and play to win games, but we will define the success of our programs beyond the scoreboard.
The SAA-SDS 5-8 athletic program allows students to discover their talents and develop their skills. The program organizes teams for varying skill levels with emphasis on participation, development, and commitment. The goal of the program at this level is to prepare athletes to compete at the varsity level. Teams participate in Shelby League, Parochial Athletic Association (PAA), Memphis Youth Athletics (MYA), Tennessee Girls Lacrosse Association (TGLA), and Shelby County Swim League (SCSL).

IMPORTANT REGISTRATION INFORMATION - PLEASE READ
Players registering after deadlines may be added to a waitlist. Team placement is not guaranteed for late registrations.
- Please register EARLY to ensure we meet required roster minimums. Registrations numbers at the deadline will determine league commitments and scheduling.
- Roster maximums are in place for some sports. If registration numbers support more than 1 team, player evaluations will be held to determine team assignments.
- If registration numbers exceed the roster maximums but are insufficient for an additional team, player evaluations will determine team placement.
- Registered families will have access to online team stores to purchase fan/player gear and apparel throughout the summer. Individual orders are processed at the time of purchase, and shipped based on your purchase date. Store information will be shared via TeamSnap.
- Athletes may participate in 1 school-sponsored team sport per season. Commitment to a team and teammates requires attendance at all practices and competitions. Any multi-sport participation MUST be cleared through the Athletic Department administration.
- JH Cheer - Practice and games schedules have been coordinated to allow girls in 7th & 8th grade to participate in JH Cheer and play a fall sport.
Player evaluations and practices for some FALL sports begin the week of JULY 27. Please make note of relevant dates.

Interested in spring golf through the Tennessee Middle School Golf Association (TMSGA)?
Open to grades 6th-8th - if there is enough interest, more info will be sent in early January re: cost, time commitment, and other logistical details.
